Trainee Feedback & Sign off

  • Sign off in Regional Anaestheisa at the RVI will only be done once we have received feedback. This form is 100% anonymous. Please complete it candidly and tell us what we are doing well as well as how we can improve. Once completed, you will receive an email acknowledgement which you can forward to Dr J Womack and request sign off. You will still need to initiate the sign off document from the e-portfolio.
